Drag and drop files here, or

Blank forms, templates, question lists or a transcript of a form-design meeting. Do not upload anything that contains personal data. Offline stand-in model (mock): nothing is sent anywhere.

    2. Generate

Rule-based stand-in. No language model is running.

    Upload a readable document first.
A saved result (adults referral form, local Gemma 3 12B). No model call.

    How it works

    1. Text is extracted from each upload (Word, PDF, text or Markdown). A meeting transcript is recognised by its speaker turns, and the model is then asked to build the form the meeting agreed rather than copy the conversation.
    2. The model is given outlines of real Mosaic forms as examples of structure and logic, a base template, and your text.
    3. It returns a form specification in JSON. The app checks it, resolves the show/hide rules against real options and writes a Mosaic .moform in the same shape as real exports.
    4. Review the preview and warnings, edit the JSON if needed, then download the .moform or the bundle for import through Template Manager.

Prototype. Every generated form needs human review before it goes near a live system. Whole forms need the model loaded with a 32K context or more.
